Notice of Intended Marriage [pdf]

The Notice of Intended Marriage (NOIM) needs to be completed, and lodged with me at least one month and one day prior to your wedding date and not earlier than eighteen months before your wedding. If you have any questions regarding completion of the NOIM, please do not hesitate to contact me.

Application for Certified Copy of Marriage Certificate [pdf]

This form is used to obtain from Births, Deaths and Marriages (BDM), Queensland a certified copy of your official marriage certificate. A fee is involved and you should contact me to find out the current cost. If you complete this form and lodge it with me prior to your wedding I can forward it to BDM along with your other marriage documentation. If you wish, you can lodge the form direct with BDM, however, you will need to supply appropriate identification documentation.

Application for Commemorative Marriage Certificate [pdf]

BDM also offers commemorative marriage certificates and samples can be viewed at their website, If you wish to purchase a commemorative marriage certificate, you should use this form and tick the boxes relating to commemorative certificates. A certified copy of your official marriage certificate is included. There is a fee involved and you should contact me to find out the current cost. Lodgement of this form is the same as for the certified copy of the official marriage certificate.

Application for Certified Copy of Birth Certificate [pdf]

If you were born in Queensland and need to obtain a copy of your birth certificate from Births, Deaths and Marriages (BDM), then you can use this form. Further information is available from the BDM website.
